Liability Waiver for ALL IN LACROSSE Participants

THIS LIABILITY WAIVER (this "Agreement") IS A RELEASE OF LIABILITY AND INDICATES YOUR ASSUMPTION OF THE DIRECT AND INHERENT RISKS ASSOCIATED WITH THE ACTIVITY.

1. Acknowledgment of Risks

By signing this Agreement, you acknowledge that participation in lacrosse involves the risk of physical injury, including but not limited to sprains, fractures, tears, and other injuries. You understand that these injuries may occur from participation in drills, games, or related activities.

2. Release of Liability

In consideration of being permitted to participate in the activity, you hereby release, waive, and discharge ALL IN LACROSSE, its owners, directors, officers, employees, agents, and representatives from any and all claims, liabilities, or damages arising from any injury sustained while participating in this activity, regardless of whether such injury is caused by the negligence of the organization or otherwise.

3. Photography Policy

You acknowledge that during the activities, photographs might be taken for promotional purposes. By participating, you agree to allow the use of your image in these photographs without compensation. If you do not wish to be photographed, please inform the staff prior to the event.

4. Emergency Contact

It is important for your safety that we keep current emergency contact information on file. Please ensure your emergency contact details are up to date and accessible during the duration of your participation.

5. Agreement to Terms

By signing this waiver, you affirm that you have read and understood the risks involved, you are aware of the possibility of injury, and you voluntarily choose to participate in the lacrosse activities offered by ALL IN LACROSSE.

By checking this box, I acknowledge that participating in lacrosse may result in potential injuries, including but not limited to tears, fractures, sprains, and other physical harm, and I accept the risks associated with such injuries. By checking this box, I agree to adhere to all safety rules and guidelines provided by ALL IN LACROSSE to minimize the risk of injury during participation. By checking this box, I understand that it is my responsibility to consult with a healthcare professional regarding my physical fitness and ability to participate in lacrosse activities, especially if I have prior injuries or health conditions. By checking this box, I grant permission for ALL IN LACROSSE to take photographs of me during events and activities, and I consent to the use of these photographs for promotional purposes, including but not limited to social media, websites, and marketing materials. If I do not wish to have my photograph taken, I will inform the staff prior to the event. I am the parent or legal guardian (or the adult athlete) and I agree to this waiver for every athlete listed above.
